Could just be that the output flange nut wasn't staked properly or backed off for some reason. Checking/torquing to spec COULD fix it, but the bearing may also be shot. hopefully it's the easy fix.

Did Torquing it back to spec fix the issue? Or does it seem that the bearing is bad too?

Bearing felt smooth once torqued to (my) spec. 😑

For anyone looking at this later, there was some slight variation to the torque spec for the "companion flange nut" depending on year, but the "safe" middle ground looks to be 90 ft-lbs.
