Getting a quote for a rebuild is kind hard if you/we aren't sure what may or may not need to be rebuilt at this point. Total engine rebuild is probably not necessary since you say it runs fine after 5 min. Transmissions are expensive to rebuild (around $2000+ average) but you'd be better off getting a used one from a salvage yard IMO.
But let's not get ahead of ourselves...like BigBallsMcfalls said, it could very well be something easy like a dirty MAF sensor (which will definitely give you those symptoms). Search for how to clean your MAF and try that 1st. I recommend you also clean your throttle body, IAC, and PCV valve while your at it. If you search, you'll find write-ups on all these topics.
Transmissions tend to slip when they are dying. Watch your RPMs, if the engine revs out while it's in gear w/o moving the truck as much as should, then you probably have trans problems. If the RPMs stay low and the truck is just sluggish - probably engine and not trans.

First thing that would be more helpful is some punctuation in your posts so we can read them.
Do you have a check engine light on?
If you do, go to an Advance Auto or Autozone and have the code pulled.
After cleaning the MAF, you should also check your spark plugs and wires if they haven't been changed in a while
