Overheating coolant problem '88 4runner

1988 4Runner, 3.0 6 cyl.

After checking my radiator coolant level the other day I did not properly secure the radiator cap. After driving a while the truck died and coolant was blasting out of the radiator, but the thermostat never went up - it stayed on normal. After letting it cool down I added water, then coolant. The truck runs ok at speed, but is really rough when starting up, and it blows smoke until it is warm.

Not sure what is going on... oil looks ok.

Any ideas?

Thanks!
Michael

Billowing white smoke out the exhaust? Time for a new headgasket.
