The good news.... with that bad of mileage, it should be some simple problem.
I would start by making sure it coasts freely on flat road, then check for fuel leaks in the tank or elsewhere.
I would definitely check the catalytic issue, try disconnecting.... are you having major loss of power?

I consistently get 260 miles / 418 kilometers out of a tank. I have a 5 speed manual transmission with the 3 liter in my 4Runner and never go above 75 mph. My friends with the auto tranny usually get about 2/3 the mileage I get as a reference point.
Do you smell gasoline? I would think that the truck would have serious fumes burning / leaking that kind of gasoline. Are you putting a full tank's worth of gas in there when the fuel gauge reads empty? I know that sounds stupid, but it could be reading empty early. I typically have to put 16.5 gallons in my truck when empty.
