I haven't been on this board in a few years. But, I'm back and feel like I should respond to this thread.
KCchick, have you checked the "cold start injector timer" to verify that it is working? That is the device that activates the cold start injector. It is located below the throttle body next to the coolant temp switch. If it is broken, worn out or screwed up, the truck will not start until the main fuel injectors reach ~50psi. Unfortunately, to get the main injectors to reach 50psi, you have to crank the engine for about 30 seconds.
So, if you have spark and air, I would definitely check this device. Used ones are about 40 bucks, new ones around 280. My engine did the same crap as yours until I asked a dealer shop foreman about it.
